Patient Goal
What they came in for.
He had been managing a failing upper dentition for six years and had been offered a full denture twice. He told us plainly that he would rather keep the problem than wear one.
Initial Condition
What we found.
Advanced generalised periodontitis affecting the upper arch, with eleven remaining teeth showing 60 to 80% bone loss and grade II to III mobility. Two had already been lost to abscess in the preceding year. The lower arch was periodontally stable with good bone. CBCT showed adequate bone volume anterior to the sinuses for a tilted posterior implant design.

A fixed, screw-retained upper prosthesis on six implants, cleansable and retrievable. All six implants integrated with no marginal bone loss beyond 0.4mm at twelve months. He returned to a normal diet at month five and reports chewing force comparable to what he remembers from before the periodontal disease took hold.

Phase 01
Prosthetic-first planning
The final bridge was designed before a single tooth was removed, and the implant positions derived backwards from it.

Clearance & placement
All eleven teeth extracted and six implants placed in one surgical session, with the two posterior implants tilted to avoid sinus grafting.

Immediate loading
A screw-retained provisional bridge fitted the same afternoon, out of occlusion on the posterior units.

Healing
Four months of integration on a soft diet, with the provisional adjusted twice as the tissue remodelled.

Final prosthesis
A milled titanium framework with individually layered zirconia crowns, designed against his lower natural arch.
Treatment process
What actually happened, in order.

Month 0
CBCT, intraoral and facial scanning, and a full prosthetic design session. The tooth position of the final bridge was agreed with him before surgery was scheduled.

Surgery day, morning
Eleven extractions and alveoloplasty under IV sedation. Six implants placed through a bone-supported guide; the posterior pair tilted 30 degrees.

Surgery day, afternoon
Multi-unit abutments placed, a pick-up impression taken, and the pre-made provisional converted and fitted. He left the clinic at 6pm with fixed teeth.

Months 1 — 4
Integration. The provisional was relined at week six as the tissue settled, and the vertical dimension checked against his lower arch.

Month 5
Verification jig, framework try-in, and a tooth try-in in wax so he could approve the final tooth position while it was still changeable.

Month 6
Milled titanium framework with layered zirconia delivered and torqued. Hygiene instruction and a water flosser, which for a full-arch case matters more than anything else.
